Repair of Complex Retinal Detachment

Patient referred from outside hospital after suffering a suprachoroidal hemorrhage during an IOL exchange procedure. Vision is light perception, IOP is 25 mmHg, and choroidals have a max height of 6 mm with a giant retinal tear and total retinal detachment on B scan.

QUESTION

Preferred management:

A. Drain the choroidals

B. Drain the choroidals and repair the RD

C. Wait for the choroidals to resolve

ANSWER

The choroidals were mainly serous and therefore observed for resolution. After 2 weeks, the repeat B scans shows max height is 2mm with a total RD and large retinal tear.
